ESA Family Acknowledgement

Please read this before you enroll. You must check the box at the bottom of the enrollment form.
This is the agreement for families who plan to ask Arizona ESA to reimburse a Dyslexia Rescue payment.

Who you are paying
1. Your contract is with Restoring Flow Ministries, doing business as Dyslexia Rescue. You pay through Stripe, the same way every other family pays. ESA families do not get a different checkout.
2. The charge is monthly. It starts on the day you enroll and renews on that same date each month, until you cancel. The price and renewal date are shown at checkout

What ESA approval means
3. Dyslexia Rescue is an approved Arizona ESA vendor. That means we are registered. It does not mean the Arizona Department of Education will approve your reimbursement.
4. You are the ESA account holder. You are responsible for your ESA contract, the category you choose, and the upload. Dyslexia Rescue is not responsible for whether ESA pays you, how much it pays, or when it pays.

How reimbursement works
5. When Stripe charges you, we will send a paid invoice for that charge. It will name the student, the service, the dates covered by that payment, and the amount.
6. You submit that invoice and your Stripe receipt to ClassWallet yourself, after the service dates on the invoice have passed. We do not submit it. We do not track it.
7. Do not also submit that same period through ClassWallet Direct Pay. One payment path only. Submitting both can pay the same charge twice.
8. We need the student’s legal name to put on the invoice. If you do not give it to us, we cannot send an ESA invoice.

No refunds
9. There are no refunds. This includes a denial, a delay, or a partial payment by ESA. Access continues for the period you already paid. ESA rules also bar a provider from refunding or rebating ESA funds to a parent.
10. To cancel, email president@dyslexiarescue.com. We cancel the next renewal the same day we receive the email. Cancellation does not refund the current period.
11. A failed, reversed, or disputed Stripe payment suspends access until it is paid. A chargeback ends the subscription.

What the service is
12. Dyslexia Rescue provides Orton-Gillingham structured literacy instruction, an online parent course and community, and, if you enroll in them, private lessons. A reading assessment we provide is an academic literacy assessment.
13. It is not a psychological, neuropsychological, or medical evaluation. It does not diagnose. It cannot be used to qualify a child for an ESA disability category.
